Severe acute pancreatitis between systematic inflammatory response syndrome and sepsis: insights from a mathematical model of endotoxin tolerance
نویسندگان
چکیده
In order to evaluate the degree of endotoxin tolerance, expressed by the reduced cytokine/releasing capacity of the whole blood, data from a group of patients with trauma, severe acute pancreatitis (SAP), and diffuse peritonitis were analyzed. In SAP endotoxin levels and the tumor necrosing factor (TNF)– releasing capacity of the whole blood under lipopolysaccharide (LPS) stimulation were of an intermediate degree between systemic inflammatory response syndrome and severe sepsis. A mathematical model of ordinary differential equations of LPS signaling based on endotoxin kinetics and endotoxin tolerance was constructed. The mathematical model was used to reproduce the TNFproduction in trauma, SAP and peritonitis patients. The results of these numerical simulations are very similar to the determinations in real patients and argue that endotoxin tolerance may be a component of the immune dysregulation that complicates the clinical evolution of the patient with SAP. © 2007 Excerpta Medica Inc. Acute Pancreatitis (AP) and Dietary Habbits
Inflammation of pancreatic parenchyma is said to be pancreatitis. It can be acute, presenting as emergency with short history or chronic as a continuation of acute with a long history. Autodigestion is responsible for pancreatitis on background of premature activation of pancreatic enzymes within the pancreas. Acute Pancreatitis is acute inflammation of the pancrease which can be mild to modera...
متن کاملA Mathematical Model of Acute Inflammatory Response to Endotoxin Challenge
The body responds to endotoxins by triggering the acute inflammatory response system to eliminate the threat posed by gram-negative bacteria (endotoxin) and restore health. However, an uncontrolled inflammatory response can lead to tissue damage, organ failure, and ultimately death; this is clinically known as sepsis. Pro-inflammatory mediators (e.g., interleukin-6 (IL − 6) and tumor necrosis f...

The clinical course of an episode of acute pancreatitis varies from a mild, transitory illness to a severe often necrotizing form with distant organ failure and a mortality rate of 20-40%. Patients with severe pancreatitis, representing about 15-20% of all patients with acute pancreatitis, need to be identified as early as possible after onset of symptoms allowing starting intensive care treatm...
متن کاملPersonalized Mathematical Model Predicting Endotoxin-Induced Inflammatory Responses in Young Men
The initial reaction of the body to pathogenic microbial infection or severe tissue trauma is an acute inflammatory response. The magnitude of such a response is of critical importance, since an uncontrolled response can cause further tissue damage, sepsis, and ultimately death, while an insufficient response can result in inadequate clearance of pathogens. A normal inflammatory response helps ...
